ACCESS IV Program for Prevention
The Argus Community Status-Neutral Care Coordination Program (SNCCP) reaches out to, identifies, and serves “high-risk” and “at-risk” HIV negative adolescents and adults with no or limited engagement in the healthcare system, are inconsistent with care, or have never or not recently been tested for HIV. The program also helps identify collaterals of known HIV index clients with histories of Intravenous Drug Use (IVDU), unprotected sex and/or unknown sero-status, almost all challenged by personal or socio-economic barriers to care.
We target and case-find people immediately upon testing at our site, as well as offering free mobile transport to and from all partner primary care providers (PCP) and linked PrEP/HIV testing sites. Program participants include sex partners of those with HIV who are ineligible for our Health Home (HH), un/under-insured, or are eligible metropolitan area/district public health offices based members not yet connected to the system (e.g., those who are undocumented and undiagnosed or recently incarcerated, men who have sex with men (MSM), unhoused transgendered persons, homeless persons, and sex-workers).
Caring Preventive staff provides integrated care to fit your individualized needs.
Services provided in conjunction with Public Health Solutions, the NYC Dept. of Health and Ryan White supported federal funds.
ACCESS IV serves the general high-risk population ages 16 and up who are interested in HIV Pre-Exposure Prophylactic (PrEP) and Post-Exposure Prophylactic (PEP). Individuals must reside in the Bronx, Manhattan, or Queens.
